Palliative Care Nurse Opportunity
The role of a Palliative Care Nurse in our service is to provide holistic care to patients across diverse communities.
Key responsibilities include:
* Providing outpatient and inpatient support to palliative care clients.
* Collaborating with interdisciplinary teams, including Occupational Therapists, Social Workers, Dieticians, and Oncology specialists.
* Supporting patients and families in making informed decisions, including advanced care planning.
* Ensuring care is compassionate, culturally sensitive, and tailored to each individual's needs.
