Overview
Step into a role where your nursing skills, your culture, and your heart genuinely matter. This is an exciting chance to grow your career as an Aboriginal Health Registered Nurse by stepping into a Clinical Nurse Consultant Grade 2 position with the Integrated Chronic Care for Aboriginal People Program. If you're passionate about community, connection, and empowering mob to live stronger, you'll feel right at home here.
Where you'll be working
You'll be based on Wonnarua Country at Cessnock District Hospital, working out of a shared office with a tight-knit, family-like team. You'll be side by side with an Aboriginal Health Practitioner and alongside the supporting ICAPP CNC, plus a chronic disease RN who's always up for a yarn and a problem-solve. The wider ICCAPP crew has your back too, offering extra support, guidance, and connection whenever you need it. It's the kind of team where you never feel like you're doing things alone.
What The Role Looks Like
Your days will take you into the community, supporting Aboriginal people across Cessnock, Muswellbrook, Singleton and Maitland. You'll be the expert nurse who brings chronic care out of the hospital and into people's lives. Expect a mix of clinical assessments, case management, care coordination, and working closely with local GPs and primary health networks to keep care flowing smoothly. No two days are the same. One morning you might be reviewing a care plan with a patient recently discharged from hospital. In the afternoon you might be chatting with a GP about a complex case or linking someone with the services that help them stay on track. It's meaningful work with a personal touch, supporting mob to manage chronic disease in a way that's culturally respectful, practical, and genuinely empowering.

If you're ready to step up, grow your career, and help shape better health outcomes for mob, this is your chance to shine.
