About the Role:
As a Personal Care Worker, you will work collaboratively with nursing staff to provide effective clinical and emotional support to residents. Your role will involve delivering high-quality, personalized care to enhance the wellbeing of residents, including those with complex needs such as palliative care, dementia support, restorative care, and pain management.
* You will coordinate personal care for residents in accordance with care plans, which meet their individual needs and preferences.
* Deliver high-quality, person-centered care to residents and families, considering cultural, spiritual, and individual needs.
* Support nursing staff regarding assessment, care planning processes, documentation, and evaluation of care outcomes.
* Monitor the cleanliness, safety, environment, and promptly report issues of concern.
Requirements:
To be successful in this role, you will need:
* Certificate III in Individual Support, or equivalent (Aged Care)
* Experience in residential aged care/home care (desired)
* Experience with dementia care and palliative care support (desired)
* Demonstrated ability to support residents in a fast-paced environment with exemplary communication skills in a team and one-on-one interactions with all levels of care partners
