**About The Role**
We are looking for talented Registered Nurses to assist in the coordination of the Chronic Heart Failure clinics. These clinics are located at Dandenong, Doveton and Berwick.
The role manages the client referral, appointments and care of the client within the clinics, communicates with the appropriate health professionals and services and follows up the plan of care. This role also involves attending to the nursing tasks that pertain to client care from the clinic, such as follow up of investigations and test results.

**What you need**:
Essential:
- Demonstrated recent experience as a Registered Nurse
- Current AHPRA registration
- Australian working rights
- Valid Driver's licence
Desirable:
- Previous Community Nursing experience
- Recent acute experience
- Post-graduate qualifications in critical care or similar
