**About the role**
An exciting new opportunity to join our thriving community health care organisation and work within a truly supportive and values based culture and engaged workforce.
We are now seeking a Community Social Work Team Leader to deliver, in collaboration with the Service Manager, a new Community Outreach program. They will lead Community Engagement Officers and Community Health nurse; to facilitate supportive connections with clients who are experiencing isolation, marginalised housing or homelessness, and/or mental health, AOD and help them connect with health services and social activities, within local community areas of Boroondara, and Manningham.
**What you will be doing**
- Directly reaching out to community members outside of the office in creative ways that meet community need
- Establish, support and supervise the new Community Outreach team
- Prioritise needs of the service recipients and maintain focus of program resources
- Staff management and success in fostering a safe and prosperous work culture
- Promote the program via community and support networks
- Monitor performance against budget
- Manage complaints and ensuring compliance with AccessHC grievance process
- Work closely with the Access to Community Program Coordinator and Access to Community Volunteer Workforce Coordinator to support each other's programs
- Participate in program evaluation and strategic improvement
- Internally negotiate smooth referral pathways for the team to support hardly reached clients easy access to services
**What you will bring**
Key Selection Criteria
- Proven experience empowering adults to improve health and wellbeing outcomes particularly with diverse and disadvantaged communities
- Demonstrated experience in stakeholder engagement with proven ability to strategically engage, negotiate and influence, others to achieve outcomes
- Experience in mental health or AOD highly regarded
- Experience and knowledge of outreach work practices and risk assessment
- Experienced in staff supervision
- Demonstrated experience in leading a diverse team of professionals
- Demonstrated ability to work in a team environment
- Effective time management and organisational skills
- Tertiary qualifications in social work, or related discipline
- Minimum of 4 years' experience in a community or social work role within a community organisation
- Proficiency in Microsoft Office and relevant software; Trak Care, Snap Forms, MS Teams, Internal Programs
- Systems knowledge (e.g. NDIS, Psychiatric services, Medical, AOD)
