About the Role
This part-time opportunity is open to a range of Allied Health Professionals, including Dietitians, Occupational Therapists, Physiotherapists, Social Workers and Speech Pathologists. Working alongside an existing clinician in a supportive environment, you will contribute to clinical and systemic disability navigation across MLHD services, while supporting key initiatives to improve compliance with state and federal policy and accreditation requirements. Location is negotiable across the MLHD footprint, offering genuine flexibility to suit your lifestyle.
What You Will Do:
In this role you will lead clinical and systemic disability navigation across MLHD, identifying and developing appropriate pathways for people with disability and complex needs to support safe discharge from hospital and prevent unnecessary admissions.
Your key responsibilities will include:
* Developing, implementing and evaluating initiatives to maximise outcomes for patients eligible to access the NDIS
* Serving as the key contact and escalation point for all NDIS and disability related matters within MLHD
* Building collaborative relationships across internal and external stakeholders to optimise patient outcomes
* Providing guidance, leadership and advice to services on matters relating to the NDIS and disability care
* Supporting proactive care navigation for people with disability in the community with unmet or unstable needs
* Building the capacity of MLHD staff to meet the health needs of people with disability
* Coordinating governance and reporting of NDIS related processes across a range of care settings
About You:
To be successful in this role you will bring:
* Current AHPRA registration as an Allied Health Professional, or a tertiary qualification in an allied health discipline with eligibility for membership of the relevant Australian professional association
* Recent clinical experience working with people with disability, their families and carers, and NDIS participants, with a demonstrated understanding of current challenges in this space
* Strong interpersonal and communication skills, with a proven ability to engage, consult, negotiate and collaborate with a wide range of stakeholders
* Demonstrated experience leading the design, implementation and evaluation of complex health or disability related projects and initiatives
* Experience in clinical teaching or supervision, with the ability to build workforce capacity
* The ability to work both independently and as part of a multidisciplinary team, with strong organisational and prioritisation skills
* An unrestricted NSW driver's licence and willingness to travel within the MLHD footprint, including occasional overnight stays
