Key Responsibilities
* We collaborate with clients to develop individualised support plans that meet their cultural and wellbeing needs.
* We assist clients to improve their independence by supporting living skills, transport, social activities, medication and personal care when required.
* We build professional relationships and networks with customers, families, caregivers and key stakeholders.
You'll Need
* Technical literacy and proven ability to use Microsoft Office and systems for reporting.
* Cert IV in Mental Health or Disability or equivalent industry experience.
* Availability to work flexible shifts across a rotating 24/7 roster.
* Nsw drivers license and ability to drive large cars and vans.
* Prior to commencing, you will need to obtain a police check, WWCC & NDIS Check.
