The Role
You'll be the primary point of contact for a caseload of Home Care Package customers across the North - West Tasmania assessing their needs, coordinating their services, managing their budgets, and visiting them regularly to make sure their care is genuinely working for them.
* Conduct intake, initial, and ongoing assessments of aged care‐related needs
* Develop and maintain goal‐directed care plans within each customer's home care budget
* Coordinate and facilitate supports and services across the Home Care Packages program
* Manage package spend and advocate for customers' needs and preferences
* Conduct regular home visits and ongoing monitoring across the Bendigo region
* Build strong relationships with customers, families, carers, and community stakeholders
About You
You bring clinical or case management foundations, genuine empathy, and the ability to work independently across a regional caseload.
* Qualification in Case Management, or an undergraduate qualification in Nursing, Allied Health, or Social Work (essential)
* Prior case management experience, particularly in home care or community services (highly regarded)
* Experience completing initial and annual customer assessments and care planning
* Knowledge of the Home Care Packages program and the broader home care sector
* Division 1 Nursing registration (desirable but not essential)
* Based in Tasmania with a current driver's licence and willingness to travel across the area
* National Police Check (or willingness to obtain)
* Proof of working rights in Australia
