About the role
The Child Health Outreach Program Nurse/AHP is integral to the Child Health Outreach Program working collaboratively with children 0 – 15 years and their families to facilitate and support children with complex chronic healthcare needs through coordination of care to optimise health outcomes. The position has a strong clinical focus on outreach work including liaison with internal and external stakeholders involved in the child and family's care.
About you
* Current registration as a Registered Nurse or Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA);
* Demonstrated understanding of primary health care and developing clinical knowledge of chronic disease management and/or child health;
* Working knowledge of Child Health and an understanding of current practices, theories and values.
* Understanding and commitment to Aboriginal health, and the philosophy and practice of Aboriginal Community Control and knowledge of current issues affecting the lives of Aboriginal People.
