What's on offer
Fulltime permanent role based in the northern suburbs (parttime will be considered).
Monday to Friday 9am - 5pm shifts, with no weekend work.

What we're looking for
You have passion with purpose and are driven by excellence. Caring is a profession and a calling, and your work and support enriches everyone. Along with your current AHPRA nursing registration and clinical expertise, you'll have:
Strong understanding of aged, community, and health care, including service coordination and compliance.
Skilled in assessments, risk identification, and providing education to clients and families.
Ability to deliver personal care and train staff in complex care tasks to ensure quality and safety.
Exceptional interpersonal skills to negotiate, influence, and resolve sensitive issues.
Tech-savvy, autonomous, and collaborative, with a commitment to outstanding customer service.
The role
Support SAH clients with complex, high, and moderate care needs through tailored, person-centred solutions.
Conduct strengths-based assessments and create care plans that empower clients.
Provide supervision and training to care staff, ensuring competence in complex care tasks and compliance with best practice.
Advocate for clients, resolve issues, and act as a trusted voice for clients and their families.
Collaborate with health professionals, manage emergencies, and find solutions for complex client challenges.
