* Clinical Care Services: We are seeking a compassionate Registered Nurse to join our multidisciplinary team, providing clinical care to clients in their homes.
* Client-Centred Approach: Embody a person-centred approach, recognising and promoting individual goals and preferences for each client.
* Care Planning: Ensure thorough clinical assessment, planning, monitoring, and evaluation of care delivered to meet client needs.
Key Responsibilities
* Deliver high-quality clinical care in the home environment, prioritising client safety and well-being.
* Collaborate with multidisciplinary teams to develop and implement client care plans that align with their goals and preferences.
* Ensure accurate and timely documentation of client assessments, care plans, and outcomes.
Requirements
* Current registration as a Registered Nurse with the relevant health authority.
* Demonstrated experience in community-based care, preferably in a similar role.
* Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare professionals.
