An exciting opportunity to join the Gippsland, VIC region with Aspire4Life as a Clinical Assessor. This community-based role involves traveling throughout the region to work collaboratively with clients in their homes and in Residential Aged Care Facilities. You will use your clinical skills to conduct assessments with residents and clients.
We offer both part-time and full-time permanent contracts, working Monday to Friday — no weekend or shift work required .
To ensure you feel confident and supported, we provide a comprehensive two-week training and orientation program. The first week is in-person at our Coolangatta office, with travel, accommodation, and meals fully covered as per our travel policy. Our next orientation begins 28th April 2025 .

The Role:
* Working independently within your region, utilizing your assessment experience to conduct holistic Home Care and Complex Aged Care Assessments with a client-centered approach focusing on wellness and reablement
* Capturing individual client information, working collaboratively to develop support plans and goals
* Supporting older Australians to live independently, considering supports such as assistive technology, social services, and community care
* Traveling to Aged Care Facilities to conduct Residential Aged Care Funding Assessments
* Using assessment tools within the My Aged Care portal
About You:
* Experience in assessment and goal-oriented support planning in aged care, community, health, or social services
* Confident communicator with a positive, collaborative approach with clients, families, and stakeholders
* Local knowledge and ability to network within your region
* Advanced IT skills, experienced with multiple platforms (Microsoft Office)
* Experience working with KPIs to ensure high-quality client outcomes
* Ability to manage time effectively in an autonomous environment
What to Bring:
* Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with 5+ years of experience
* Minimum 5 years' experience in aged care
* Evidence of influenza vaccination (for 2024 Flu Season)
* Evidence of COVID-19 vaccinations (minimum two doses + booster)
