Job Title: Advanced Care Practitioner
We are seeking a skilled and experienced healthcare professional to join our Specialist Palliative Care in Aged Care Services (SPACE) program as an Advanced Care Practitioner.
As a key member of our multidisciplinary team, you will provide expert clinical support, coordinate care, and enhance quality of life for patients with life-limiting illness.
The successful candidate will have the opportunity to work autonomously, apply critical thinking, and provide leadership in clinical decision making. You will also be responsible for advance care planning, education, and research within the SPACE program.
