We are seeking an Aboriginal Identified Case Manager to join a well-established community service, providing strengths‐based support, advocacy and case management to clients across Melbourne's western region.
About the Role
In this role, you will:
* Deliver client‐centred case management and support.
* Conduct assessments and develop tailored support plans.
* Assist clients to access housing, health, legal and community services.
* Advocate for positive client outcomes.
* Build and maintain relationships with key stakeholders and community services.
* Maintain accurate case records and reporting requirements.
About You
You will bring:
* Experience in community services, family services, housing, mental health, family violence or a related field.
* Strong case management and engagement skills.
* The ability to work collaboratively while managing a varied caseload.
* Excellent communication and relationship‐building skills.
* A current Victorian Driver's Licence.
* The ability to obtain relevant screening and compliance checks.
