Healthcare Advocate Opportunity
We are seeking a dedicated and compassionate individual to join our team as a Healthcare Advocate. This is an exciting opportunity to make a meaningful impact in the lives of patients and their families.
In this role, you will work closely with healthcare professionals to provide culturally sensitive and comprehensive care management services. You will work with each patient for approximately 6 months, helping them to overcome barriers and navigate the healthcare system.
* This is a full-time position, working 40 hours per week, Monday through Friday, from 8:00 AM to 4:30 PM.
* The successful candidate will be required to participate in a hybrid work arrangement, spending up to 2 days per week working remotely after initial training.
Required Skills and Qualifications:
1. High school diploma or equivalent required; experience working in a social service or healthcare setting preferred.
2. Valid driver's license and clean driving record required if driving is necessary for the role.
3. Two years of experience working in a healthcare or social service setting preferred.
4. Must be a resident of the community being served with good knowledge of local resources.
5. Basic computer skills preferred.
