About the Role
We are seeking two part time compassionate and dedicated Care Finders to join our team in Logan Central. In this role, you will provide personalised assistance to older Australians—particularly those from culturally and linguistically diverse (CALD) backgrounds—to navigate the aged care system and access essential services.
This is a fantastic opportunity to support isolated, vulnerable, or hard-to-reach older people facing significant barriers in accessing aged care and related support. You will provide intensive, person-centred assistance including needs assessment, service facilitation, and ongoing support. Assertive outreach to actively engage individuals who may not otherwise seek help is a key part of this role.
You will work using culturally safe, trauma-informed approaches, fostering strong community and stakeholder connections to ensure equitable, respectful, and tailored care that upholds the dignity and autonomy of CALD clients.
About You
You are passionate about supporting older people from culturally and linguistically diverse (CALD) backgrounds to navigate the aged care system with dignity and respect. You have strong communication and interpersonal skills, enabling you to build trust and rapport with vulnerable individuals and their communities.
With a solid understanding of My Aged Care and related support services, you are skilled at conducting culturally sensitive, holistic assessments and providing tailored guidance. You thrive working collaboratively with community groups, service providers, and health professionals to ensure seamless care coordination. Your commitment to cultural competence, client advocacy, and accurate record-keeping underpins your ability to deliver person-centred, trauma-informed support that makes a real difference.
Key responsibilities include:
- Assist CALD older people to understand and access My Aged Care and related services
- Conduct holistic, culturally sensitive needs assessments
- Proactively engage in outreach to build relationships with older individuals facing barriers
- Link clients with appropriate aged care providers and supports, facilitating service agreements and ongoing access
- Develop and maintain networks with community groups, service providers, advocacy organisations, and health professionals
- Maintain accurate and confidential client records for evaluation and reporting
- Apply a person-centred, respectful, and culturally safe approach
- Collaborate with multidisciplinary teams to ensure coordinated care delivery
Desirable Criteria:
- Prior experience working with older people or in the aged care, health, or community services sector.
- Demonstrated experience supporting people from culturally and linguistically diverse (CALD) backgrounds, including migrants, refugees, and new and emerging communities.
- Experience in case work, care coordination, or community navigation roles.
- Proven experience in community engagement and outreach within multicultural or vulnerable populations.
- Familiarity with data collection, reporting, and outcomes measurement in a service delivery context.
- Lived experience within a CALD community and/or fluency in a language other than English.
