This Team Leader position is responsible for providing leadership and clinical oversight within Community Care Services, ensuring high-quality, person-centred care for people with complex needs and older individuals living in the community. The role involves supervising and supporting Care Partners, managing service delivery operations (including client allocation and workload distribution) and contributing to strategic planning alongside the management team. Key aspects include fostering strong partnerships across health and community sectors, driving service improvements, monitoring program performance and ensuring compliance with organisational goals and funding requirements. The Team Leader also supports staff development, conducts performance reviews and maintains accurate reporting and documentation.
