0re seeking someone who is both caring and capable someone who thrives on connection and brings a high level of professionalism to everything they do.You will ideally have:Experience in care coordination, case management, or a similar roleA background in aged care, disability support, or community servicesExcellent communication, time management, and problem-solving skillsA strong ethical approach and attention to detailConfidence using technology and care management systemsA current driver2 a0
