Aged Care Assessor Role
We are currently seeking a skilled and compassionate Comprehensive Assessor to play a vital role in assessing the care and support needs of frail older people.
Key Responsibilities:
* Conduct comprehensive assessments of individuals' care and support needs, ensuring high-quality person-centred outcomes.
* Develop and implement effective care plans that meet the individual's goals and objectives.
* Collaborate with multidisciplinary teams to ensure seamless care transitions and enhance patient experience.
* Communicate effectively with individuals, families, and carers to facilitate informed decision-making.
Requirements:
* Relevant tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist, Exercise Physiologist, Social Worker, Speech Pathologist, Dietitian, or Psychologist.
* Current registration with the relevant Australian Health Practitioner Regulation Agency (AHPRA) where applicable.
* Demonstrated high-level verbal and written communication skills, including negotiation and conflict resolution abilities.
* Ability to work independently and collaboratively within a team environment.
* Effective prioritisation and organisation skills to manage multiple responsibilities.
