About the Role:
The Nurse practitioner role is part of a nurse led team in the North Canberra Hospital (NCH) Geriatric Rapid Acute Care Evaluation (GRACE) Service. The GRACE Nurse Practitioner (NP) provides advanced nursing care both autonomously and in collaboration with GRACE Medical Officers and GP's. Recognised as an expert practitioner and clinical leader, the NP possesses advanced knowledge, skills, and competence within their credentialed scope. This role involves promoting nursing by exploring and expanding the boundaries of nursing practice through evidence-based and best practice literature. The NP demonstrates clinical excellence in contemporary nursing practices, offering expert advice and identifying opportunities for quality improvement.
GRACE is an outreach service that provides a point of liaison and clinical support and education at the Residential Aged Care Facilities (RACH)/Acute care setting interface. The GRACE Service liaises between Residential Aged Care Home (RACH) residents, their family and carers, GPs, Emergency Department (ED) staff, inpatient units of North Canberra Hospital, Canberra Hospital and ACT Ambulance Service (ACTAS), for improved care delivery to acutely unwell residents in the ACT.
In addition to clinical support, The GRACE service provides a formal education program for RACHs including a standard suite of sessions / subjects. Analysis of specific needs of each RACH is regularly undertaken, and education is tailored to the specific needs of the RACH.
The primary aim of the nurse practitioner position in the GRACE service is to provide advanced assessment and clinical management of residents of Residential Aged Care Home (RACH) to prevent avoidable presentations to public Emergency Departments (EDs).
The GRACE Nurse Practitioner will continue to develop/evolve to meet the changing clinical requirements of both GRACE and the RACHs.
