We are seeking a highly skilled and experienced professional to lead our NDIS Local Area Coordination team.
This position plays a pivotal role in supporting people with disabilities, their families and carers, and the broader community, ensuring individuals with disabilities are included and valued, able to participate in and contribute to community life, and connected to resources, services and support they need to achieve their goals.
About the Role
* Servicing five Local Government Areas of Coffs Harbour, Bellingen, Nambucca Heads, Kempsey and Port Macquarie
* Supporting eligible people with disability to access the NDIS and implement their NDIS plan
* Assisting NDIS participants, their families and carers to identify and pursue goals that enable participants to live a good life
* Builing the capacity of participants, their families and carers to exercise choice and control, and to manage their NDIS plan and lives as independently as possible
* Builing the capacity of people with disability who are not eligible for the Scheme, their families, carers and broader circles of support to generate options and implement solutions, and to access other services and supports within the broader service system and community
* Builing community capacity for greater inclusion of people with disability, and creating beneficial linkages within the community to expand options and possibilities for people with disability
Key Responsibilities:
1. Support participants in developing and implementing their NDIS plans
2. Develop and maintain effective relationships with NDIS participants, their families and carers
3. Work collaboratively with internal and external stakeholders to ensure seamless service delivery
4. Maintain accurate records and reports to meet organisational requirements
5. Participate in ongoing professional development to enhance skills and knowledge
