Our care partners play a vital role in delivering individualised services to our customers in their homes, ensuring they can remain living their optimal lives, safely at home for as long as possible.
Job Description
* The Care Partner performs an integral role in assisting our customers with their daily needs, supporting them to live independently and maintain their health and wellbeing.
Key Responsibilities
1. Undertake assessments of customers in their homes to identify their needs and develop care plans that meet those needs.
2. Ensure follow up to reduce risks, refer allied health professionals, order equipment etc and evidencing customer progress.
3. Ongoing case management supporting customers in their clinical and holistic needs, monitoring and communicating on their wellbeing and reporting on progress.
4. Documenting customer goals, progress and outcomes using person centred, wellness, reablement and enabling strategies.
5. Undertake administrative duties including data entry, processing invoices, following up customer enquiries and ensuring documenting all interactions to ensure program guidelines are followed.
6. Ensure evidence is gathered and uploaded including quotes, confirmation of delivery of products and services.
7. Liaise with relevant community based agencies and service providers to ensure an integrated and cohesive approach to service delivery.
8. Assist in customer service schedules including service planning, confirmation and short notice placements or cancellations.
9. Work with customers with their budgets and monitor customers' budgets in accordance with program guidelines.
10. Communication with key stakeholders, internally and externally.
11. Community Engagement: engagement with key community stakeholders, promoting our organisation's values and mission.
12. Documenting all evidence requirements in line with compliance, ensuring 100% compliance.
13. Oversee and manage customers' budgets (Government Funding).
14. Working with or under the guidance of medical practitioners and allied health professionals.
15. Ensure that all services are provided in a caring and respectful way in accordance with our organisation's policy and procedures, legislative requirements and program specific guidelines.
16. Provide training, coaching and guidance to colleagues as required.
17. Conduct skills assessments of colleagues performing complex care services as required.
18. Demonstrate a commitment to best practice and evidence of continued professional development.
19. Participate in staff meetings and attending training opportunities when required.
20. Occasional wound care (Clinical Care Partner only).
21. Liaison with Aged Care Assessment Team (ACAT), My Aged Care (MAC) Portal, PRODA and other regulatory bodies.
22. Assist customers to access other alternative services where the services are no longer meeting their needs.
23. Identify and participate in personal professional development.
24. Apply and uphold principles of equity and anti-discrimination in the workplace and adhere to organisational and legislative Health, Safety and Environment requirements.
25. Audit customer files regularly to ensure compliance.
26. Demonstrate a commitment to best practice and evidence of continued professional development.
27. Any other duties as required to meet the ongoing needs of our organisation.
