We are seeking a dedicated Professional Care Coordinator to provide essential lifestyle, social and wellbeing services for our residents.

About the Role
">
* Encourage resident choice and participation in individual and group activities
* Assist in the design, delivery and monitoring of group and individual lifestyle services and activities
* Implement innovative events and lifestyle activities for residents and where possible involving the wider community, volunteers and residents' family and friends
* Provide direct resident care during activities such as bus trips, including assisting with personal hygiene and toileting
* Provide direct resident care in relation to meals service, mobility and social support
* Support your peers and maintain a cooperative team spirit
* Work in a safe manner to protect yourself, your colleagues and residents closely following all policies and procedures

Requirements
">
* A kind heart and genuine passion for providing high-quality care to the elderly
* Strong communication skills and a positive attitude
* Certificate III or Cert IV in Leisure and Health or Individual Support or similar
* Able to establish and maintain professional and appropriate relationships with residents and their families
* A reliable team player
* Commitment to respecting the values, customs, preferences and spiritual beliefs of residents
