Homes Support Worker

About the Role:
Care partners play a pivotal role in supporting older adults to live independently and well in their own homes.
The role requires a deep understanding of aged care services, a client-centred approach, and strong communication and coordination skills.
You will work closely with clients, families, and multidisciplinary teams to develop and implement personalised care plans that align with each client’s unique needs, preferences, and goals.
Key Focus Areas:
* Assessing Client Needs:
o A comprehensive assessment of individual client needs, including clinical requirements and personal preferences, to develop tailored care plans that meet service expectations
* Coordinating Care Planning:
o Effective coordination and oversight of client-centred care planning to ensure that services are appropriate, responsive, and person-focused
* Service Delivery Management:
o Monitoring and managing the delivery of in-home support services to guarantee compliance with Aged Care Quality Standards
* Client Relationships:
o Maintaining strong client relationships through ongoing support, clear communication, and guidance throughout the care journey
* Team Collaboration:
o Collaborating with multidisciplinary teams to document, review, and ensure compliance with care plans and related activities
