Bidgerdii Community Health Services is an Aboriginal and Torres Strait Islander community controlled primary health care service, operating in the Rockhampton and wider Central Queensland region for more than 30 years.
Our services include general practice bulk-billing clinics, social and emotional wellbeing and generalist counselling services, chronic disease management, preventative health screening, children and family health and early childhood education services
About the Role:
The Elders Care Connector has the primary responsibility of connecting with local Elders, older Aboriginal and Torres Strait Islander people and their families to raise awareness of their aged care entitlements. The overarching function of the ECC is to ensure that support for clients is organised, sequenced, and supported to enable effective access to appropriate aged care services both internal and external to the (ACCHO).
The ECC will work closely with the Elders Care Support Coordinator. They will also help determine the level of local demand for aged care services, identify existing aged care services, any service gaps in the area and continue advocacy work with clients as required.

About You:
To be successful in this role you must have:
* No qualification is required for this role - Training will be provided
* Excellent and strong relationships with the local Aboriginal and Torres Strait Islander Community.
* Good communication and interpersonal skills, with the ability to build trusting relationships in community.
* Demonstrated understanding of the principles of person-centered care and trauma-informed practice.
* Experience working with older people.
* Understanding of the internal ACCHO services available to clients.
* Knowledge of or willingness to learn about the aged care landscape.
* Ability to or willingness to learn how to enter data and reporting.
* Demonstrated experience working unsupervised and as part of a team.
Practical Requirements:
* Current C Class Drivers Licence (QLD) – essential.
* Occasional travel – within Bidgerdii's service areas (Including Mt Morgan and Central Highlands as required).
* Immunisation requirements – It is a condition of employment for this role for the employee to be, and remain, vaccinated against the following vaccine preventable diseases: Measles, Mumps, Rubella, Varicella, Hepatitis B, Hepatitis A and Pertussis (Whooping cough) during their employment.
* Current CPR and First Aid Certificate.
* Current Blue Card (Working with Children and young people) or willingness to apply for a Blue Card.
* Satisfactory Police Criminal History Check.
