Case Manager - Mental health Professional - Federal Government Department $65 - $73.85 an hour Super Government Department role, long term contract Open to Psychologists, Social Workers, Mental Health Nurses, Occupational Therapists and Counsellors ACM Healthcare is working on behalf of a Federal Government Department to source an experienced Mental Health Professional for a Clinical Care Coordinator role, providing Case Management services to veterans. This is initially a 24 month contract at an APS6 level. Duties: Deliver high-quality, evidence-based clinical services, including risk assessment, brief interventions, care coordination, and referrals for clients with complex/multiple needs. Provide short-term, goal-oriented, clinical case management intervention to clients who present with unmet health and psychosocial needs that impact on their ability to access and/or meaningfully engage in mental health care. Maintain accurate and confidential client records, including progress notes, care plans, and risk assessments. Collaborate with internal teams and external organisations to ensure coordinated, client-centred support and effective referral pathways. Work within a stepped-care model, supporting vulnerable or at-risk communities and families. Contribute to service improvement initiatives, program delivery, and community engagement activities as required. Mandatory Requirements: Qualifications as a registered Psychologist / Mental Health Occupational Therapist / Mental Health Registered Nurse with full AHPRA registration, OR Qualification as a Social Worker with membership of the Australian Association Social Workers (AASW) on commencement OR A Masters-level counselling qualification and registration with PACFA as a Registered Clinical Counsellor or with ACA at Level 3 or Level 4 Working with Children (WWC) and/or Vulnerable People Checks/Clearance must be obtained prior to commencement. As this role is within a Federal Government department it is a required to be an Australian Citizen at time of commencement The successful candidate will be required to obtain and maintain a Baseline Vetting (AGSVA) security clearance.
