This specialist plays a pivotal role in supporting young people transitioning from custody to the community. The primary focus is on coordinating post-release care for individuals with mental health and/or drug and alcohol concerns.

Your Key Responsibilities:

* Develop comprehensive care plans, deliver targeted interventions, and promote engagement with relevant health and support services.


Required Skills and Qualifications:

* Aboriginal descent or identification as an Aboriginal person, with acceptance within the community.
