Are you passionate about improving the health and wellbeing of people in our community through social connection? Amity Health is seeking a motivated Link Worker to join our team and support participants to access community groups, programs, and services that enhance social engagement and quality of life.
About the Role
As a Link Worker, you will play a vital role in supporting participants to identify their goals and social needs, and connecting them with appropriate services, programs, and networks. Your responsibilities will include:
* Using motivational interviewing techniques to understand participant goals.
* Building and maintaining knowledge of referral pathways and community resources.
* Keeping accurate records and maintaining service mapping databases.
* Fostering collaborative relationships with stakeholders and the wider community.
* Promoting the role of social prescribing within the region.
* Contributing to program objectives through timely completion of tasks, supporting the organisational team, and ensuring program compliance and reporting.
About You
We are looking for someone who brings experience, compassion, and strong relationship-building skills. You will ideally have:
* Qualifications and/or experience in aged care, human services, or a related field.
* Experience working effectively with vulnerable or marginalised groups (e.g., migrant or Aboriginal communities).
* Proven ability to provide client-focused care.
* Knowledge of aged care and related community services.
* Strong data entry and portal navigation skills.
* Excellent community engagement and relationship-building abilities.
Mandatory Requirements:
* Current 'C' Class Driver's Licence.
* Current National Police Clearance.
* Current Working with Children Check.
