ABOUT THE ROLE

We are seeking a passionate Home Care Manager to join our team.

The Home Care Manager will be responsible for leading the strategic development and delivery of Evergreen’s Support at Home service, driving growth, quality and innovation while enabling clients to maintain independence and choice in their own homes.

As a key member of the Executive Leadership Team, the role shapes service delivery, operational performance and long-term sustainability of a contemporary, person-centred home care model.

Evergreen is currently scaling its Support at Home service and is seeking an experienced leader to drive growth and service delivery.

Evergreen is open to full time and permanent part time (30.4 hours per week).

Key Responsibilities:

* Provide strategic and operational leadership to establish, grow and optimise a high-performing home care service aligned with Evergreen’s vision and HEART values.

* Lead the development and delivery of an innovative home care model tailored to retirement village environments, driving service growth, efficiency and exceptional client experiences.

* Ensure strong governance, financial performance and reporting, including the establishment and monitoring of KPIs and identification of sustainable commercial opportunities.

* Oversee compliance with legislative, contractual and Aged Care Quality Standards, embedding effective risk management systems, policies and safe work practices.

* Foster a high-performance culture through effective people leadership, workforce planning, performance management and staff development.

* Drive continuous improvement through stakeholder engagement, feedback and innovation to enhance service quality and outcomes.

* Collaborate across the organisation and with external stakeholders to implement enabling systems (including IT) and support organisational priorities and change initiatives.

ABOUT YOU

You are a caring and motivated leader who is passionate about supporting older people to live with dignity and independence. You bring a person-centred approach to care and take pride in leading, developing and growing a team to deliver high-quality services in a collaborative environment.

You will possess:

* Relevant tertiary qualifications in Health, Social Sciences or a related discipline

* Proven experience (3+ years) in delivering or managing Home Care services

* Demonstrated leadership experience within Home Care, Regional/Home Care Operations or similar environments

* Experience successfully establishing, growing or transforming home care services

* Ability to lead and manage a multidisciplinary team (including clinicians and administrative staff), with oversight of clinical practice, service delivery, and the operational and administrative functions of a home care service.

* Strong commercial acumen, including experience managing budgets, resources and workforce planning

* Ability to think strategically and translate vision into practical operational outcomes

* Collaborative and engaging leadership style, with a demonstrated ability to motivate and inspire teams

* Strong commitment to quality, compliance and continuous improvement in service delivery

* Current National Police Check (or willingness to obtain)
