About This Role
This role is key to our Home Care team in Lindisfarne, Tas. You will work with clinicians, case managers and clients to deliver high-quality care.
Key Responsibilities:
* Deliver patient care as part of a multidisciplinary team.
* Collaborate with healthcare professionals to achieve optimal outcomes for clients.
* Maintain accurate records of client interactions and progress.
Required Skills And Qualifications:
Clinical experience in aged or disability care preferred but not essential;, a compassionate approach to caring for others;, relevant tertiary qualifications; registration as an RN (Aged Care) highly desirable.
