The Role
We are seeking a passionate and experienced Wellbeing Caseworker to join our team. In this role, you will be responsible for case managing Aboriginal children and young people, ensuring their unique needs are met through tailored support services.
 * Deliver culturally appropriate case management services to clients, focusing on individualised care and support.
 * Collaborate with multidisciplinary teams to ensure seamless service delivery, promoting client outcomes and reducing barriers.
 * Assist clients in accessing community resources and support services, empowering them to take control of their lives.
 * Advocate for clients' needs within the healthcare system and community settings, driving positive change and systemic improvement.
Maintain accurate and timely records in compliance with organisational policies and procedures, ensuring transparency and accountability.
Requirements
 * Certificate IV in Community Services or related fields, demonstrating a strong foundation in case management and cultural competence.
 * Demonstrated knowledge and understanding of issues affecting Aboriginal communities, including cultural sensitivities and historical context.
 * Experience working within a case management model, with a proven track record of delivering effective and compassionate support services.
 * Proficiency in report writing, with excellent communication skills and attention to detail.
