Clinical Care Partner

In this rewarding role, you will work alongside older people with higher clinical needs, supporting them to live well and maintain their independence while receiving government-funded aged care services.

Key Responsibilities
* Facilitate clinical assessments, risk evaluations, and develop individualised care plans.
* Coordinate and monitor services to ensure they meet clinical goals and client needs.
* Work collaboratively with GPs, allied health professionals, support workers, and families.
* Support clients with transitions between care settings and future planning, including emergency preparedness.
* Maintain accurate, timely, and audit-ready records and documentation.
* Promote reablement and restorative care strategies that reduce service dependency.
* Identify risks and ensure safe clinical practice in line with governance standards.
* Provide weekend and public holiday on-call support on a rotating roster.
About You
You are an empathetic, motivated professional with a passion for helping older people live independently and achieve the best quality of life. You thrive in a team environment, can manage complex cases, and balance compassion with strong clinical skills.
* Bachelor's degree in nursing or occupational therapy (or similar community-based clinical qualification).
* Previous experience in community health, aged care, or allied health.
* Strong understanding of aged care reforms, Support at Home framework, and clinical governance.
* Experience in assessment, case management, and person-centred care planning.
* Excellent communication skills with the ability to engage diverse stakeholders.
* Current First Aid Certificate, driver's licence, and own insured vehicle.
