About the Role
As a Community Nurse (RN or EEN) you will provide professional, person‐centred nursing care to clients in their own homes across Support at Home, CHSP and private programs.
Your work helps people maintain their independence, manage their health needs, and continue living safely and comfortably at home. You will deliver a mix of clinical care, health monitoring, education, and care coordination, working collaboratively with a multidisciplinary team.
This role covers Melbourne's North, West and East/South‐East, with flexibility in assigned coverage areas and is a 12‐month fixed‐term contract.
What You Will Be Doing
- Deliver high‐quality nursing care aligned with individual needs and care plans.
- Conduct holistic clinical assessments and elevate concerns appropriately.
- Provide wound care, medication management, chronic disease support and post‐hospital recovery care.
- Monitor health status, document accurately and prepare clinical reports.
- Work collaboratively with clients, families, carers and health professionals.
- Promote independence, dignity, choice and well‐being.
- Participate in continuous improvement and uphold Aged Care Standards.
- Support new staff and contribute to a positive team culture.

What You Bring
Essential
- Registered Nurse or Endorsed Enrolled Nurse with unrestricted AHPRA registration.
- Experience conducting holistic nursing/clinical assessments.
- Knowledge of aged care legislation, Aged Care Standards, OHS and food safety requirements.
- Strong communication, compassion and client‐focused practice.
- Ability to work independently with initiative and sound judgement.
- Excellent time management and organisational skills.
- Proficiency with IT systems and clinical documentation.
- Valid driver's licence.
Desirable
- Experience in community aged care or health services.
- Understanding of SAH/CHSP/STRC/DVA programs.
- Additional training in areas such as continence, dementia, wound care, diabetes or medication management.
