Clinical Care Coordinator and Nurse Mentor

The role of Clinical Care Coordinator and Nurse Mentor is an exciting opportunity to join our team in Ipswich. This full-time position reports to the General Manager and requires strong clinical expertise and leadership skills.

Key Responsibilities:

* Provision of clinical guidance and leadership of care teams to ensure high-quality care for residents.
* Collaboration with the Care Manager to lead carer teams in delivering optimal clinical care outcomes.
* Management of assessment, documentation, and submission of AN-ACC requirements within the facility.
* Support for effective documentation and user support of policies, procedures, and IT system requirements.
* Coordination of assessment, care planning, and review processes.
* Support for the admission of new residents and initial consultations with residents and families.
* Accurate recording and maintenance of organisational and legislative requirements.
* Input and oversight into facility team rosters and skill mix availability.
* Management and documentation of clinical risk and commitment to continuous improvements.

Requirements:

* Minimum 12 months experience as a Clinical Care Coordinator or similar leadership position.
* Innate empathy for residents and commitment to continually improving the quality of clinical care.
* Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
* Post graduate qualifications in aged care advantageous but not essential.
* Experience and knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
* Engaging style with ability to influence and develop clinical leadership skills.
* High level of computing skills for documentation and reporting requirements.
* Confident team player with excellent communication and interpersonal skills.
* Ability to manage multiple stakeholders and have a balanced approach to emergency situations.
* Valid NDIS Workers Screening Clearance.
