Job Description:
We are seeking a highly skilled Social Coordinator to join our team on a full-time basis. This rewarding role involves collaborating with community clients to design and implement wellbeing and social programs that enhance their quality of life.
This position requires strong interpersonal skills, empathy, and understanding of issues affecting older individuals, their families, and carers. The successful candidate will work in partnership with Care Partners to ensure person-centred services are delivered, monitor program expenditure, and support staff delivering/assisting with social and wellness programs.
Required Skills and Qualifications:
* Cert IV Leisure and Health or Diploma of Community Services
* Demonstrated ability to work in a holistic, person-centred model of care
* A commitment to supporting older people to remain at home in the community
The ideal candidate will possess excellent communication and organisational skills, with the ability to work independently and as part of a team.
