Job Description
RSR Consulting Erma Care Pty Ltd is seeking a qualified and experienced Community Worker to provide case management, community support, and advocacy services to individuals and families experiencing social, emotional, and practical challenges. This is a full-time, permanent role based in Ashgrove, Queensland, responsible for delivering structured support programs and coordinating community services in accordance with ANZSCO 411711 – Community Worker. Benefits Attractive salary package of AUD $77,000 – $80,000 per annum plus superannuation.

Task & responsibilities Interview clients and assess the nature and extent of social, emotional, financial and practical difficulties. Assess the needs of individuals and families and develop structured support and recovery plans. Plan, implement and review life-skills, educational and training programs that promote independent living.
Provide practical and emotional support and refer clients to emergency relief and community resources, including food, housing and welfare services. Monitor client progress and maintain accurate case documentation, progress notes and reports. Identify and respond to immediate risks including mental health crises, housing instability and health emergencies.
Assist with recovery planning, goal setting and access to mental health and wellbeing services. Advocate on behalf of clients to access health, welfare, employment, housing and training services. Assess community needs and available resources for health, welfare, housing and employment services.
Liaise with community groups, welfare agencies, government bodies and service providers to ensure coordinated support. Promote awareness of community resources and strengthen community engagement initiatives. Support families, young people and vulnerable individuals to address social, emotional and financial challenges.
Encourage participation in meaningful community activities to promote healthy and active living. Coordinate voluntary and emergency support services, including outreach and food assistance programs. Provide transport assistance to appointments, programs and community-based activities where required.
Qualification & experience Diploma or higher qualification in Community Services, Social Work, Welfare, or a related field. Minimum one (1) year of relevant experience in community services, case management or support coordination.
Demonstrated ability to assess client needs and develop structured support plans. Strong written documentation and case management skills. Ability to liaise with government bodies, welfare agencies and community stakeholders.
