We are seeking an experienced Mental Health Clinician to provide high quality community based, mental health assessment and management service to people requiring mental health care in the
**Community Mental Health program in a Community Mental Health Team in Berri, South Australia (3 hours from Adelaide).** This position is responsible for a distinct case load and provides outreach to specified communities as well as locally. The position is part of a multidisciplinary team that operates under an integrated framework with a focus on recovery oriented practice and patient centred care. Engagement with community and non-government services is an important component of the role with an expectation to lead the development of innovative service options.
**Key Responsibilities**
- Provide interventions case management to a range of Adult clients who are referred
- Provide suitable referral pathways to clients and their families/carers
- Work in collaboration with key stakeholders and the local community
- Provide safe clinical practice and professional advice with experience in Case Management and Triage.
- Provide mental health assessment, care planning, implementation and evaluation service
- Using best practice principles as part of a collaborative partnership with consumers, families, the multidisciplinary team and other key stakeholders.
- Provide Outreach services to other rural and remote areas in South West.
- Record appropriate professional reports, statistics and maintain up to date clinical records.
**About You**
- Completed Qualification in Social Work
- Eligibility with AASW
- Current Drivers Licence
- Current Working With Children Check or eligibility to obtain
- Full Vaccination evidence including 3 x covid vaccinations and influenza.
**Remuneration Package**
Generous Agency Rates based on your experience up to $65 per hour + Super + Free Accommodation + Travel Allowance.
