* We are recruiting experienced support coordinators to join our team at Pivot Support Services to provide NDIS services to our community.
* Reporting to the newly appointed NDIS manager
Introduction:
* As a Support Coordinator within our NDIS team, you will play a vital role in empowering participants to understand and implement their NDIS plans, connect with appropriate services, and build the supports they need to thrive independently in their communities.

The role:
* Providing client centred therapy, assessment and support planning
* Advocate for client needs and ensure they receive high-quality, person-centred care
* Achieving and maintaining organisational set KPIs
* Service provider travel to clients in the community, school or home
* Meet compliance requirements for the NDIS such as timely support plans, report writing, service agreements and other
* Clinical case note recording for all service provisions
* Provide assistance to link clients, their families and carers to supports available in the community
* Liaise with service providers, community groups and other stakeholders to arrange and monitor client services
* Regularly review and update client support plans to reflect changing needs and circumstances
* Identify opportunities to improve service delivery
What we're looking for
* Relevant tertiary qualification in community services, disability support or a related field
* Experience in Support Coordination and/or NDIS is preferred
* Strong understanding of the disability support sector and the NDIS
* Excellent communication, interpersonal and advocacy skills - verbal and written
* Ability to work collaboratively with a diverse range of stakeholders
* Strong organisational and time management skills
* Contribute effectively to fortnightly team meetings and be a team player
* Be self-motivated, efficient and committed to optimal client care
* Have a growth mindset and able to hit the ground running
* Current driver's license
* NDIS Worker Screening Check or willingness to undergo screening
