About the Opportunity
You will deliver comprehensive nursing services across diverse community and functional settings while making a meaningful difference in people's lives.
Working within a supportive multidisciplinary team, you will develop and implement holistic care plans, manage complex clinical cases including wound care, and provide leadership and supervision to ensure consistently high-quality care delivery.
Key Responsibilities:
* Clinical assessments, care coordination, family liaison, and facilitating smooth transitions between community and respite care settings.
* Collaborate closely with the Clinical Nurse Manager and Clinical Nurses to provide person-centred care that empowers clients to achieve their best possible quality of life.
About You
You will thrive in team environments, building strong working relationships with diverse stakeholders, from Home Support Team Members to external service providers. As a compassionate advocate, you can navigate complex biopsychosocial challenges while empowering clients to make informed decisions about their care and maintain their autonomy.
Requirements:
* Registration as a Nurse with AHPRA
* Current Certificate of Registration Status
* National Police check
* Australian Drivers licence
* CPR and Manual Handling
