Clinical Care Coordinator | Busselton location | Full-Time, Monday to Friday

About the Role:

Step into a pivotal clinical leadership role where your expertise shapes the standard of care and makes a meaningful difference every day. As a Capecare Clinical Care Coordinator, you will lead the delivery of safe, high-quality, person-centred care, ensuring services are delivered in partnership with consumers and aligned with best practice. You will establish and maintain systems and support staff to deliver care that is compliant with the ACQSC Quality Standards, reflects Capecare's Clinical Governance Framework, and brings Capecare's Purpose, Vision and Values to life. Through your leadership, you will foster a culture of accountability, continuous improvement, and excellence, empowering teams to deliver care that meets and exceeds expectations for residents, families, and the broader community. This role works alongside a team of Clinical Care Coordinators and reports to the Clinical Manager.

Your key responsibilities will include:

* Care Coordination:
o Attend huddle and handover to ensure accurate and timely communication between shifts and identify and complete follow-up actions.
o Support and facilitate Registered and Enrolled Nurses to ensure holistic, individual assessment of residents.
o Engage consumers and nominated representatives in planning care; conduct agreed services and care plan consultations/reviews on schedule.
o Coordinate Resident of the Day Assessment schedule.
o Communicate relevant information to nominated representatives as escalated by Registered Nurses, including open disclosure discussions.
o Monitor the medication management system and support staff in its use.
o Escalate to the Clinical Manager any incident or change in a consumer's health status that may indicate increased risk, including reportable infectious diseases, critical incidents, or compulsory reporting events.
o Ensure all AN-ACC reviews are undertaken on the required schedule.
o Act as the first point of contact for resident families and representatives when issues cannot be resolved by the RN.
o Support staff in the required use of the Care Management System and Medication Management System.
o Ensure all clinical documentation is completed accurately and on time.
o Ensure accurate and comprehensive investigations of consumer incidents are completed.
o Understand the reporting requirements of SIRS and ensure timely reporting of incidents as required.
* Education and Training:
o Take personal responsibility for updating knowledge base of contemporary nursing issues.
o Attend mandatory training and proactively pursue professional development.
o Maintain and update knowledge of aged care quality standards, Capecare Clinical Governance Framework and associated practices.
o Develop and deliver formal and informal education to nurses, carers and other staff, including onboarding and toolbox sessions.
o Mentor nurses to support the acquisition of new skills and consolidation of clinical knowledge and practice.
* Leadership and Staff Management:
o Provide clinical leadership to Registered Nurses and the Care team.
o Participate in recruitment and probation reviews of Registered and Enrolled Nurses, Team Leaders and Care staff as directed.
o Contribute to onboarding of Registered Nurses via the Buddy program.
o Oversee daily staff allocation to ensure an appropriate skill mix.
o Provide guidance and feedback to Registered Nurses, Enrolled Nurses and Carers and take immediate action to rectify poor care practices.
o Ensure appropriate allocation and supervision of work-experience/students and Capecare Trainee placements.
o Assist with the completion and investigation of staff incident forms.
* Quality:
o Contribute to Capecare's Plan for Continuous Improvement.
o Undertake data collection for monthly audits (internal and external) and clinical indicators, including the National Quality Indicator Program.
o Participate in the development and delivery of action plans for audits, risk reports and risk registers.
o Identify potential breaches of regulatory compliance and escalate to the Residential Manager.

To be successful in this role, you'll need:

* Minimum of 2 years of clinical leadership experience.
* AHPRA Registration
* Demonstrated senior leadership with experience in aged care or a related field
* Demonstrated knowledge of the Aged Care Act and Standards
