Care Coordinator - Aged Care Opportunity
We are seeking a dedicated and experienced Care Coordinator to lead the delivery of high-quality clinical care for residents with complex needs.
About the Role:
As a key member of our care team, you will oversee resident assessments, develop tailored care plans, and coordinate clinical documentation and compliance reporting. Your leadership skills will enable you to mentor a team of RNs, care staff, and volunteers, ensuring seamless care delivery.
You will be responsible for monitoring and improving clinical programs and quality indicators, championing a culture of safety, wellbeing, and continuous improvement.
This is a pivotal leadership position, reporting to the Service Manager, with direct responsibility for clinical staff and indirect influence across the broader care team.
Requirements:
* AHPRA Registration as a Registered Nurse (RN) with a current Practicing Certificate
* Minimum 3 years' post-graduate experience in aged care or acute clinical settings
* Proven leadership and workforce management skills
* Strong clinical assessment, care planning, and documentation capabilities
* Intermediate computer literacy (Word, Excel, Outlook, PowerPoint)
* A commitment to quality, safety, and continuous improvement
* Current National Police Check – or willingness to acquire
* Current First Aid & CPR certification – or willingness to acquire
Desirable Qualifications:
* Postgraduate qualifications in Gerontology, Dementia, or Aged Care
* Experience with mobile clinical documentation systems
