South Rural Primary Care Network are seeking a medications monitoring coordinator to join their teams, working across 5 of our practices. This role is offered on a part-time basis for 20 hours per week.
Main duties of the job
* Assist the doctors and other clinicians in the provision of a high quality service
* Support the PCN pharmacy team with high risk drug monitoring
* Support delivery of QOF, incentive schemes, QIPP, Investment and Impact Fund (IIF) and other quality or cost effectiveness initiatives
* Support the delivery of structured medication reviews working with the clinical team
* Work collaboratively within the team to manage patients needing medication reviews and support the delivery of this work to ensure all patient groups are reached
* Help design efficient templates, protocols, alerts and other IT tools to support safety, patient care, quality record keep keeping and reporting
* Carry out audits to identify improvements and maximise income
* Work with the GPs and other primary care professionals to identify and manage a caseload of patients as and when required.

Person SpecificationExperience
* Previous primary care experience is an advantage
* Sensitive and empathetic
* You will need access to your own transport and the ability to travel across the locality on a regular basis.
* Experience of using PAM desirable
