North and West Remote Health (NWRH) is currently seeking an adaptable Journey Coordinator within the Stepped Care service system to join our team.
The Journey Coordinators are dedicated positions within the Stepped Care service system that operate as an independent partner in a person’s care journey. They may be standalone services, delivered within a Front Door service, or as an adjunct to other Stepped Care services. The role ensures individuals are at the centre of their own health and wellbeing, leading to improved care and mental health outcomes.
Your primary responsibility will be to deliver high-quality support and services by improving outcomes for Stepped Care participants, building psycho-social education, supporting access to self-management approaches including digital mental health services and group programs, and facilitating the Stepped Care model to ensure quality, continuity, and coordination of care between service providers.
This is a full-time role based in Cairns with outreach travel required.

About you
We seek individuals with the following qualities and experience:
* Ability to establish, develop, and maintain productive relationships and networks
* Experience in partnerships and collaborations that enhance outcomes and improve the service system
* Understanding of mental health challenges and their impact on individuals
* Ability to develop genuine relationships with clients accessing Stepped Care
* Recovery-focused, person-centred, strengths-based, and trauma-informed approach
* Minimum Certificate IV in a mental health-related qualification (only those with suitable qualifications will be considered)
