About the Role:
* We are seeking a skilled professional to work with vulnerable populations, including children, young people, and their families. The ideal candidate will have excellent communication skills and be able to build strong relationships with clients and communities.
Key Responsibilities:
* To provide tailored support and advocacy services using a strengths-based approach
* To conduct comprehensive assessments and develop, implement, and review safety and support plans
* To coordinate wrap-around support by working closely with government departments, schools, health services, and community agencies
* To identify and respond to risks, including escalating concerns and making reports where necessary
Requirements:
* A proven track record of working with vulnerable populations and building strong relationships with clients and communities
* Excellent communication and interpersonal skills
* The ability to work independently and as part of a team
* Knowledge of local community resources and services
