The focus of the IUIH Aged Care team is to work with the local Indigenous community and relevant key stakeholders to increase access to comprehensive care and provide well-coordinated, quality in home care services to eligible aged care clients.About the RoleThis position plays a key role within an innovative pilot program delivering culturally safe, person-centred aged care Assessment services to older Aboriginal and Torres Strait Islander people, operating in one or both Caboolture and Sunshine Coast Aged Care Planning Regions (ACPRs).
The role provides leadership to a multi-disciplinary team delivering the innovative Assessment Services to older Aboriginal and Torres Strait Islander people (Pilot).
You will be responsible for overseeing home support and Comprehensive Aged Care Assessments of Aboriginal and Torres Strait Islander older persons while complying with policies and procedures of the Aged Care Single Assessment System.We have a full-time temporary role available.
Within this role, you will be required to:Acquire and maintain a thorough understanding of the Assessment policy and procedure documents which govern the Aged Care Single Assessment SystemManage referrals following the Best Practice Steps outlined in the My Aged Care Assessment Manual.Assess referrals and determine the appropriate outcomeManage and oversee the activities of aged care assessors and ensure that all mandatory training required for the Aboriginal and Torres Strait Islander Aged Care Assessment Pilot is completed by assessors within required timeframes.Liaise with the Administration Officer, ensure rostering and scheduling of assessment services is logistically sound, productive and effective use of staffing resources.About YouCurrent C Class Drivers Licence (Qld)Current Criminal History Check – required prior to commencementCurrent Disability Worker Screening Check (QLD Yellow Card) – required prior to commencementTertiary qualification in a health-related discipline directly related to health, aged care or related specialist area.At least one year's Aged Care Needs Assessment experience is preferred.Demonstrated experience in the coordination of Aged Care service delivery and/or Aged Care Assessment TeamsDesirable, but not mandatoryKnowledge of Aboriginal and Torres Strait Island culture through either identifying as Aboriginal or Torres Strait Island person and/or demonstrated experience of working successfully within Aboriginal and Torres Strait Island communities.Knowledge of current Australian Aged Care System reforms and access, assessment, service delivery arrangements.What Can we OfferAt IUIH we recognise that we operate in a diverse community and welcome and encourage applications from all ages and genders, Aboriginal and Torres Strait Islander peoples, culturally and linguistically diverse groups, the LGBTIQ+ community, veterans, refugees, and people with disabilities.At IUIH 'Our Collectivity is Our Strength'.
