VMCH – At Home Aged Care | Melbourne
VMCH is seeking an experienced Community Physiotherapist to join our At Home Aged Care Allied Health team on a 6‐month fixed term contract.
This is a community‐based role, supporting older people to remain independent and safe in their own homes through high‐quality, client‐centred physiotherapy services.
Key responsibilities
* Deliver physiotherapy assessments and interventions in clients' homes
* Develop and implement goal‐focused treatment plans
* Support mobility, balance, reablement and overall safety
* Complete professional documentation and reports
* Work collaboratively with clients, carers and internal stakeholders
About you
* Tertiary qualification in Physiotherapy with current general AHPRA registration
* Ability to work independently in a community setting
* Strong communication, organisation and time‐management skills
* Current Victorian driver's licence and willingness to travel
* Interest or experience in aged care or community health
