Salary & Benefits
* RN Level 3 Award Rate: $51.21/hr – Base Salary: ~$99,859 + Super.
* Salary packaging available (increase your take‐home pay!).
* Mix of company car use and use of own car with mileage reimbursement.
* Salary Sacrifice – Not for Profit option.
The Role
Hold a caseload of 6 aged community clients with complex clinical and psychosocial needs.
Provide advanced clinical oversight and care planning.
Review and update care plans across the broader service (~50 clients).
Work closely with and report to the Home Care Manager.
Support care delivery for clients with multiple comorbidities, cognitive and behavioural considerations, and diverse living environments, requiring adaptability and a holistic approach.
What You'll Bring
* Current AHPRA registration as a Registered Nurse.
* Experience managing complex cases, ideally within community, mental health, or vulnerable populations.
* Strong skills in clinical assessment, care planning, and coordination.
* Confidence working autonomously while contributing to a collaborative team.
* A genuine passion for equitable healthcare and social impact.
* Aged Care experience is ideal, with knowledge of the Support at Home program.
