We are seeking a motivated and experienced Physiotherapist to supportolder people to live safely and independently in their own homes across various locations in SoutheastTasmania.

In this role, you will provide client-centred physiotherapy services across the community, helping individuals maintain their independence, safety, and engagement in daily activities. You will work closely with clients to understand their goals, assess their needs, and develop practical strategies to support everyday function and wellbeing.
You will deliver evidence-based interventions in home and community settings, including functional assessments, equipment prescription and environmental modifications. Working collaboratively with care partners and other health professionals, you will contribute to coordinated, person-centred care and support the delivery of high-quality services.
With the ability to manage your own caseload and work across a community setting, you will play a key role in promoting reablement, reducing risks, and supporting clients to live well at home.
Key responsibilities include:
* Conducting comprehensive in-home assessments of functional capacity, mobility, safety and independence needs
* Working with clients to identify goals and develop individualised, goal-based care plans focused on reablement and independence
* Recommending and prescribing assistive equipment, home modifications and adaptive strategies to support safe daily living
* Delivering evidence-based physiotherapy interventions in home and community settings
* Supporting clients to maintain or regain independence in activities of daily living and participation in meaningful activities
* Educating clients, carers and support workers on safe practices, equipment use and self‐management strategies
* Collaborating with care partners, nurses, GPs and allied health professionals to coordinate care delivery
* Maintaining accurate, timely and compliant clinical documentation and reports
* Identifying, managing and reporting risks, incidents and clinical concerns in line with organisational processes
* Managing a community‐based caseload, including planning travel and meeting service delivery requirements
* Participating in quality improvement activities, audits and ongoing professional development
Skills and Experience:
* Tertiary qualification in Physiotherapy
* Demonstrated experience in aged care, community health or a related setting
* Strong assessment, clinical reasoning and report‐writing skills
* Ability to work independently, manage a caseload and prioritise workload effectively
* Excellent communication and interpersonal skills in a client‐focused environment
Mandatory:
* Current registration with AHPRA
* Working with Vulnerable People registration
* Current National Police Check
* Willingness to travel across various locations in Southeast Tasmania
* Experience in Support at Home or aged care services
* Knowledge of reablement and restorative care approaches
