Utilise your administrative expertise to provide support and advice to Corrections Victoria Ongoing, Full-Time opportunity to work with a high-performing and passionate team in the Victorian Public Service VPS Grade VPSG3: $81,496 - $98,955 plus superannuation. About the Business Unit: Western Plains Correctional Centre is a modern, maximum-security prison focussed on rehabilitation, housing up to 1248 people in custody. Western Plains features multi-purpose infrastructure which is adaptable to the evolving needs of Corrections Victoria and the people in our care. Modern amenities include medical wards, a cultural centre, and vocational training facilities. Western Plains provides a holistic, multidisciplinary approach to individualised care which motivates, encourages and promotes reintegration into the community. Western Plains has advanced technology which improves security, streamlines operations and enhances connections for the people in our care. Western Plains operates with three primary service teams: Workforce and Prison Services, Communities, and Safety and Operational Service which collaboratively deliver comprehensive services for people in our care, safety management, and corporate services. Through its innovative design and service model, Western Plains aims to create a thriving environment conducive to safety and positive behavioural change. About the Role: The Assessment and Transition Co-ordinator (ATC) forms part of the multi-disciplinary Offender Services team, which is responsible for the development, co-ordination and delivery of a range of programs and services across several specialist portfolios: transition and reintegration, family engagement, disability, diversity and complex care. The ATC is responsible for the administration of the Corrections Victoria Reintegration Pathway, including co-ordination and facilitation of prisoner risk and needs assessments, transition and discharge planning, parole application processes, and referrals to a range of pre and post release services. The ATC works directly with people in care to assist transition into, through, and out of prison; collaborates with multi-disciplinary teams to ensure integrated case planning and case management practices; and supports the service delivery of contracted providers. In addition, the position will contribute to and support the location to embed a positive pro-social environment, and a connected, multi-disciplinary approach to rehabilitation. The ATC is responsible for ensuring compliance with requirements as outlined within the prison's Service Delivery Outcomes, and other relevant policies. Some of your duties will include: * Support the delivery of the Corrections Victoria Reintegration Pathway in a prison environment. * Administer prisoner risk and needs assessment tools to effectively identify rehabilitation and reintegration requirements, making referrals to relevant programs and services as required. * Work collaboratively with multi-disciplinary teams to develop integrated and effective transition and other specialist support plans, including actively involving and motivating prisoners to engage in goal setting and program/service participation.
