About the role:
We are looking for a passionate and experienced part-time Support Coordinator to join our dedicated team in Ballarat, working 3 days a week. In this role, you will empower participants to connect with and maintain the supports outlined in their NDIS plans. You will also build and nurture strong professional relationships with participants, their families, and other service providers to ensure seamless and effective support.
Applicants should have 2 years of experience in Support Coordination and have a sound knowledge of the NDIS and local services. This is a part-time role 3 days a week, days and times can be negotiated with the successful applicant. Option to work from home & office, once competent in our processes and comfortable in your new role.
Key Duties and Responsibilities:
* Provide experienced Support Coordination within the NDIS framework
* Implement and manage individual NDIS plans effectively
* Monitor participant progress and adjust support as needed
* Prepare and submit regular reviews and reports to ensure quality service delivery
Applicants must have prior experience in NDIS Support Coordination or NDIS Planning.
Salary as per Social, Community, Home Care and Disability Services Industry Award 2010. Interviews will be held as successful applications are received
